Thank you for your interest in Saint Jude the Apostle Catholic School. There are limited spots still available for the 2019-2020 academic year. 

Application Process

The application process is considered complete once the following have been received by our admissions office:

  • Application
  • Application fee paid through PayPal ($175 – covers applicant test and test results)
  • Birth Certificate (copy acceptable)
  • Baptismal Certificate (copy acceptable)
  • Immunization Form GA3231 (copy acceptable)
  • Archdiocesan Parish Verification Form (not required for non-Catholic applicants)
    • Online form - All Saint Jude Parish families use this form
    • PDF Form - Catholic families of other parishes use this form
  • Student Evaluation Form (sent from your child’s teacher)
  • School Transcript (all grade reports and standardized testing)
  • Reports of any psychoeducational testing administered to the applicant

Your child will be assigned an applicant testing date and time upon receipt of the application and application fee. The applicant testing dates are February 22 and March 7, 2020. 

The admissions committee will review the application, applicant test results, and all additional paperwork. All decision letters will be mailed on April 3, 2020, and an email will be transmitted on April 4, 2020.

Submit to the applicant’s current school:

  • Student Evaluation Form Grades Kindergarten – 1st
  • Student Evaluation Form Grades 2-8
  • School Transcript/Release of Records

Submit to the current parish of record:

  • Parish Verification Form
